BEFORE YOU ARRIVE

New Patient Forms
Complete your new patient forms online or arrive 15 minutes early to fill them out in the office.

ID & Insurance
Bring your photo ID and dental insurance card if applicable.

Medications List
Prepare a list of current medications, medical conditions, and any physician or specialist contact information you would like us to have.

Your Questions
Write down any questions or concerns you want to discuss with the doctor.
DURING YOUR VISIT
Welcome
You will be welcomed by our front office team and shown to a treatment room.
Imaging
We will take detailed digital X-rays and, when indicated, a CBCT 3D scan to get a complete picture of your oral health.
Examination
Dr. Hauser or Dr. Marino will perform a thorough examination, including an oral cancer screening and periodontal assessment.
Discussion
We will sit down with you to review findings, discuss any concerns, and outline recommended next steps, with no pressure to commit on the spot.
Next Steps
Before you leave, our team will help you understand your treatment options, timeline, and any financial considerations.
